Making a referral

Health care providers who wish to refer a new patient for psychiatric services should fax a completed patient referral form to (920) 996-2214. To complete the referral process, the family must contact the intake coordinator at (920) 996-2215 to schedule the appointment.

Patients seeking mental health services will be seen by appointment only. Walk-in patients will be asked to call (920) 996-2215 to set up an appointment.

Speakers and training sessions are available

Speakers are available to discuss mental health issues that affect children and adolescents. Training sessions for people who work with children on a regular basis also are available. Learn more.
